Felony forms on OCA website

This link goes to the felony form package from the Office of Court Administration.  There is also a self-executing packed file with all forms available for downloading and unzipping. On their site it says

The Office of Court Administration ("OCA.") in collaboration with Texas criminal justice professionals has prepared and promulgated the standardized felony judgment forms pursuant to Section 42.01 of the Texas Code of Criminal Procedure.

The forms are available in both Adobe Acrobat format (.pdf) and Microsoft Word 2000 format (.doc). You can view the forms online and/or download a set of forms in either format.

It is recommended that the forms be downloaded in Microsoft Word format. These forms offer more functionality and can be customized for your county and court. Click on the Overview button for more information.

Form Modifications

OCA will modify the judgment forms as necessary to ensure correctness and completeness.  If you would like to receive an e-mail notifying you of changes, please e-mail us using the link below .  If you prefer not to be notified, we invite you to check the website frequently for changes.  The following substantive change has been made to the forms:  DS1-DS5; CS1, CS2, CS4-6. 

1/14/04 - Substance  Abuse Treatment Conditions:

HB 2668 requires community supervision with appropriate substance abuse treatment for many drug-related offenses.  A new option has been listed in the "Special Orders" section that reads as follows:

Substance Abuse Treatment Conditions:  Pursuant to Article 42.12, Section 1(c)(2), the court orders the Defendant to comply with the following substance abuse treatment conditions:  _________________________.

The Felony Forms Toolbar has been modified to include this special condition on applicable forms DS1-DS5; CS1, CS2, and CS4-6..  Click on the toolbar icon to insert the text of the special order into your judgment form.  You may then add the specific treatment conditions ordered by the court.

5/19/03

Updated forms can be downloaded from the website.  The "Applicable Punishment Range" dropdown menu has been corrected to include "Capital Felony".  The items are listed in the order of severity with drug offenses, habitual offender and repeat offender entries listed last.  The dropdown menus in the Adobe PDF files were also corrected.

Updated forms can be downloaded from the website.  The forms have been modified to enable use of the felony forms toolbar that copies the text of special orders into your judgment form.  

11/19/02

The Felony Forms toolbar contains macro buttons that will copy the text of special orders into your judgment form.  

5/13/02 - Field Formatting Changes: 

Four different forms contained fields formatted in a number format rather than in a regular text format which affected the users ability to enter both alpha and numeric data in the fields.  If you are experiencing problems due to these errors, please download the revised forms.  The forms affected are:

CS2 - Field #1 Cause Number

DS1 - Field #1 Cause Number

DS5 - Field #1 Cause Number

CS3 - Field #18 Date of First Community Service Fee Payment 

4/05/02 - Note to Word 97 users:

We've determined that the standardized forms do not work well with Microsoft Word 97.  If you are a Word 97 user and are experiencing problems, please let us know what types of problems you've experienced.  We're especially interested in hearing from users that have found solutions to problems encountered while using the standardized judgment forms in Word 97.

2/12/02:

All Forms:

  1. Punishment Range:  The dropdown menus have been expanded to include drug offenses.  Three ranges for drug offenses have been added to include:
    • 5-99 yrs and a fine not to exceed $50,000
    • 10-99 yrs and a fine not to exceed $100,000
    • 15-99 yrs and a fine not to exceed $250,000

  2. Plea to Offense:  Corrections have been made to the dropdown menu to include three options: 
    • Guilty
    • Not Guilty
    • No Contest

In addition, we've made several minor formatting changes and changes to the document view settings that do not affect the content of the forms.
